In the UK at least, I don't think the majority of fat people are fat because they don't realise eating unhealthy foods to excess will make them fat. It's because they prefer to eat them to excess than exercise the self control required to stay a healthy weight.

People everywhere continue to underestimate how unhealthy it is to be fat. Everyone knows it's bad, not everyone knows it's going to kill them 10 years early and significantly impact their quality of life after a certain age. It seems relevant enough to make documentaries about, in any case.
