I don't remember exactly, I guess you could google it, it's common to give blood data in such a form in Poland, as far as I'm concerned.
You have to drink a lot and eat a non-fat breakfast before doing it, it should make you feel a lot better.
I donated twice, I've been allowed to do it only since January (18th birthday) and the dates didn't particularly fit me for more frequent actions, but I should get back on that in the near future. It's awesome, though!

Its funny, somehow I feel like its a bad thing to have a common bloodtype. As if it has value to be more special in a way that probably wont affect your life.
